Multidimensional view on the work of children's nurse in the emergency room of children's ward

The work of nurses in the outpatient department belongs between discussed topics. The interplay and cooperation of all members of the medical team are especially important. Correct communication between all team members and communication with children and their accompaniment are also a matter of course. The aim of this diploma thesis is to map the work of a nurse in a comprehensive view in the outpatient area of the children's ward, the satisfaction of children and parents in the outpatient area of the children's ward and to suggest possible improvements for the outpatient sphere of the children's ward of České Budějovice a.s. The diploma thesis is processed by qualitatively quantitative investigation using semi-structured interviews, questionnaires and observations. The research group consisted of nurses, doctors, paediatric patients and their accompaniment at the children's ward, paramedics and the general public as well. The interviews were categorized and processed by the method of pencil and paper. Questionnaires were processed statistically. The hidden participating observation was focused on the collaboration and communication of nurses from outpatient department with other nurses, doctors, paramedics, children and their accompaniment; and it was also focused on the satisfaction of the children or their accompaniment at the outpatient department. The results of the work showed that the view of the work of the paediatric nurse is distorted both from the health care professionals and the general public, which means that the nurse is seen mainly as a nursing care provider. The results also showed the necessity of proper communication and knowledge of foreign languages. At the same time, the thesis was used to identify shortcomings or suggestions for changes in the paediatric outpatient department of the České Budějovice Hospital, including the satisfaction of patients and their accompaniment during visiting the department that depends on the time spent in the waiting room. Suggestions for improvement were drawn up, drafted and handed over to the children's ward management.
A retrospective look at the development competence of pediatric nurses on inpatient department

This thesis deals with not only a question of competence of nurses and their children's development, but also in terms of preparation of professional nurses. The nursing skills are necessary competence to perform the nursing profession. Represent this area of knowledge, skills and attitudes that are acquired through education. One of the ways to qualify nurses to the profession, is today a university course. The second method is a specialized education, provided that the candidate meets the conditions for admission and received his professional competence to practice nurses. Determination of the competencies of nurses in the Czech Republic is governed by Law No. 96/2004 Coll. Acquisition and recognition of eligibility for non-medical professions and for activities related to providing health care and amending certain related acts, the last decree on the activities of health workers and other professionals, No. 55/2011 Coll. The thesis is divided into theoretical and practical part. The theoretical cross section contains the history of pediatrics and nursing, selected chapters from health legislation and development of children's ward in České Budějovice Hospital. For the practical part, I chose a qualitative research method of interviewing and pediatric nurses. The research sample consisted of 10 nurses who work or have worked on the inpatient pediatric ward. There were set three goals in this work. The first task was to determine the development of competencies nurses. The second objective was to determine the extent of the authority nurses at present. In the last part of my job I surveyed whether nurses are aware of their general competence. The research showed that their competencies developed. A large share of this development has the technological development in medicine, new insights in treatment and use of modern diagnostic methods and increasing nursing education. Results of this research can serve for professionals in pediatric congresses and publication in professional journals.
Quantification of Basic and Special Nursing Performance on Standard Units of the Children's Department

Abstract Quantification of General and Special Nursing Procedures at Standard Units of Paediatric Departments The present thesis is divided into two parts, a theoretical and practical part. The theoretical part deals with the history of paediatrics, training and competencies of paediatric nurses, contemporary paediatric care and the range of provided services. There is a separate part describing general and special nursing care. Due to a limited scope of the bachelor's thesis, attention was paid to principal activities of paediatric nurses. Within the first part of the research survey two objectives and five hypotheses were established. The work aimed to inventory general and special nursing care at standard units of paediatric departments. The following hypotheses were set: H1: The most frequent general nursing procedure is patient personal hygiene care. H2: The most frequent special nursing procedure in the "Medication Administration" category is the oral (per os) administration of medication. H3: The most frequent special nursing procedure in the "Monitoring" category is the monitoring of body temperature. H4: The most frequent special nursing procedure in the category of "Special and Diagnostic and Therapeutic Procedures" is biological sampling. H5: The most frequent special nursing procedure in the category of "Other Patient Related Activities" is the reception of a patient. The research survey was carried out as a quantitative survey. The research sample consisted of 49 nurses of standard paediatric departments at five selected hospitals, namely the hospitals in České Budějovice (Nemocnice České Budějovice, a. s.), Prachatice (Nemocnice Prachatice, a. s.), Strakonice (Nemocnice Strakonice, a. s.), Písek (Nemocnice Písek, a. s.) and Český Krumlov (Nemocnice Český Krumlov, a. s.). The second part of the survey focused on assessing the selected nursing procedures. General nursing procedures included meal preparation, feeding and diapering a baby. Special nursing care included oral (per os) and rectal (per rectum) medication administration, also intravenous injection, infusion and its monitoring, assisting in examinations, blood and faeces sampling, taking part in the round and patient handover. Each procedure was measured ten times and the result was counted up and divided by their number in order to obtain the average value. The hypotheses were tested by means of the Kruskal-Wallis test where the mean rank value was set. On the basis of the test the set hypotheses were either confirmed or disconfirmed. In the area of general nursing care where three categories were compared, the highest value (463.68) was obtained for the nutrition care. The first hypothesis was therefore not confirmed. In the area of special nursing care, the "Medication Administration" category, the highest value (403.07) was obtained for medication preparation and oral administration. Again, the hypothesis was not confirmed. The hypothesis in the "Monitoring" category was confirmed since the highest value (539) was obtained for body temperature monitoring. Following, there are "Special and Diagnostic and Therapeutic Procedures" where the hypothesis was not confirmed as well since the most frequent procedure is the preparation of a child for an examination (662.21). Based on the arithmetic mean calculation in the last category of "Other Patient Related Activities" where the highest value (8.09) was obtained for filling out nursing diagnoses, the hypothesis was not confirmed as well. The results of the observation part served for creating a draft of a software application aimed at calculation of the nursing load in a monitored number of patients. This application is ready for further development.
The contentment with nursing care at children´s department from the view of a person accomplanying the child

Satisfaction monitoring is one of the indicators of quality of provided care. The quality of nursing care provided in hospitals has been largely discussed recently. For many health care facilities the patients´ opinion is crucial for the nursing care evaluation and represents a feedback for the nursing staff. The objective of my thesis was to examine opinions of persons accompanying child patients in hospital on the nursing care quality. I set three hypotheses: The first hypothesis is {--} persons accompanying child patients in hospital are satisfied with the care, the second states {--} persons accompanying child patients in hospital are satisfied with communication with the nursing staff in a paediatric ward and the third hypothesis suggests that persons accompanying child patients in hospital are not satisfied with the paediatric ward equipment. The research sample was formed by persons accompanying child patients in the Český Krumlov hospital,Inc. Those persons were staying is hospital together with children during their hospitalization. The research was conducted in the paediatric ward of the Český Krumlov hospital,Inc. by the questionnaire method. The research took place in January, February and March. I analysed data obtained from 69 filled in questionnaires. The data analysis shows that persons accompanying children in hospital were satisfied with the care provided {--} the hypotesis I is confirmed. The persons accompanying children in hospital were also satisfied with communication {--} the hypothesis II is confirmed, and they were satisfied with the paediatric ward equipment {--} the hypothesis III isn´t confirmed. The results of my research will be given over to the Český Krumlov hospital, Inc. management, primarily to the Chief Nursing Officer of the Hospital, to the Chief Physician and to the Senior Nurse Manager of the paediatrical ward. The results may serve as a feedback and an expression of respect for the quality of nursing care in the paediatric ward in the hospital Český Krumlov,Inc.

Úvodní část se věnuje popisu zpracovaného zdravotního zařízení. V teoreticko-metodologické části je popsána analýza nákladů a teorie bodu zvratu. V aplikační části jsou uvedeny výsledkové údaje nemocnice, jejich roztřídění, zpracování a doporučení změn pro oddělení. V závěru pak vyhodnocení a doporučení změn pro management.
